COSHH Assessment

Hi everyone,
Can anyone help me? I put COSHH data sheets on sites but have been asked by a couple of clients to provide a COSHH assessment. Trouble is I havn't seen one of these, so I'm a bit lost as to what they expect. Does anyone have one that they would let me have?

OK so does anyone know where I might be able to just look at one so I get the idea of what one looks like?
