On a first clean I go around doing the whole frame along with the glass (avoiding any vents).  Then I go around again doing glass only.  I charge extra for first cleans.
On maintenance cleans, sometimes I will do the whole frame along with the glass and sometimes I will do glass and sills only (only once around on these though).  It depends on their condition and how energetic I'm feeling.
