Examples of quotation forms

Hi Guys,

Since I'm not native English ( but Dutch), I find it extremely difficult to set up something like this, even though my English is quite good, I still have difficulties with setting things up professionally, I'd like to be able to go round properties for a quote and if they are not there just leave a quotation form...

I searched the forums, but I can't find anything to help me out...

I'm aware that I'm asking a big favor, but it would help me immensely, just sorting the final bits and pieces out  :-[ :)

you couldnt send me one could you lads as i am currently wrighting quotes on the back or buisiness cards , never thought of a quotation form.

I sent you an email earlier today with a file attached.  It's not one of my better efforts but will give you an idea.
