If you require a RAMS document to be filled out then it will be for a business.
I don't have one but this is something you will need to tailor to your job and could well be different to the next job you do.
RAMS simply means a risk and method statement.
So what risks are involved at that particular site that you need to evaluate. (All jobs will have some risk but you have to show you are aware of what they are and score it. 0 is no risk, 1 is very low risk and 5 is unacceptably high risk.)
So if you are clearing gutters from a ladder you have to evaluate your risk of hurting yourself if you fell and hurting someone else or damaging property in the process. You may score that as a 3.
Your method statement would state that to reduce that risk you would secure your ladder by tieing it off and that you will use a safety harness with a front clamp and lanyard to arrest a fall but allow you to work efficiently at the same time. You would not over reach and you will pay attention to the weather - not doing the job in high winds, or when its raining or when there is ice on the ground.
If there is foot or vehicle traffic about you should detail the risk and how you would minimise this. A solution could be to do the job over a weekend when there is no one around. Do you have another person who will act as a banksman? Do you need to cone an area off so it becomes an exclusion zone? You will put up warning signs. What PPE will you use, ie safety shoes, high visibility vest/jacket etc.
