I think there may be a few issues here  

Just to get you started;

!, One guy standing on top of velux ... er no fall protection what so ever , generally speaking that can damage the frame..
2, On previous posts I asked you how you protect wash from entering storm drainage as its not allowed to , you replied 'if it runs into the guttering you have applied too much' ...er its overflowing the guttering  
3, Not one ladder has any safety devices attached, ie in case you don't understand , no roof stand offs or base supports..

And the list goes on  

If it helps I will add a pic a day from your site with constructive comments, just to help you out
