Employment status declaration

Been asked to do some work for a company and they want an "employment status declaration"

Never come across this before, and HMRC websites not much help. Googled it and the best i can find is basically something i write up stating im self employed, and have been for X amount of years and say that i will pay my own tax etc..

Is this all they want? The guy im dealing with in the company doesnt really know what it is, as he is just ticking off things i need to supply to get on the suppliers list.

Schedule D number and NI number should be enough.
Plus PL cert etc etc that goes with your business.

I was asked to clean the carpets at my daughters school in the hols by the caretaker. I did it and submitted an invoice, 2 weeks later I got an employment status declaration from the borough council. Straight forward, just fill in that you are self employed etc, then had an email to say I was now accepted on the suppliers list and my invoice was paid. Painless, and makes it easy for next time i suppose!
