Risk Assesment & Method Statements (HELP)

Hi,

I've been asked to give a quote for a large two storey hotel.  The guy who phoned me said that if I get the job (and I do want it) the following will be required:

    Public liability for 5 million (not a problem)

    Risk Assesments (HELP)

   Method Statement (HELP)

    Etc (HELP)

Can anyone send me examples of a Risk Assesment and Method Statement?  Any help or advice here would be much appreciated.

Also - any ideas what he meant by 'Etc'?  

I didn't like to ask for fear of being found out for being the inexperienced, poor-excuse of a Geordie maggot window-cleaner I am!

Should have looked closer to home ;D
I have a copy of of a general risk assessment training prochure wrote by "opus Health and safety"you can have. It explains how to risk assess a Job and it also has a template for presentation to the client. If you want it I will drop it up to you.

etc could be COSHH data sheets, and COSHH assessments, also he may need info to be site specific.

there may also be a case of not using ladder access, therefore if cherry picker accesss, then he may be looking for proof of IPAF to cover himself in case of accident.

COSHH usually applies to chemical us and yes f**ry is a chemical. if you require COSHH details just contact the manufacturer they have to provide them by law if requested.

You usually write the method statement after you have done the risk assessment. And is basically a statement to show how you intend to avoid the risk

Example of Method Statements for a Tender. or Contract, may be as follows:

Working at Heights (HA6)

Hazards

Risk Rating

Danger of personnel falling High

Danger of falling objects to personnel below High

Planned Methods To Reduce Risks

* Suitable access will be provided to the working area, and shall be inspected at regular intervals.

* All operatives working on site shall wear safety helmets at all times.

* Signs and barriers will be positioned directly below the works to warn and prevent access to the working area.

* All current legislation and codes of practice concerned with working at heights, will be strictly complied with.

* Training and instruction will be given to staff on the usage of lines and harnesses.

This is very Basic and broad, as Martin19842 wrote they could ask to be more site specific i.e. accessing a particular area. and you could go on about avoiding customers, staff etc

I quickly hacked this method statement together from another source  so don't take it literally word for word there are many ways to right a method statement just think of the every tasks from start to finish like you are going to explaining to an allien then turn it into a written document.

I know it doesn't help you now but we will soon have generic method statements and risk assessments available for you to download.

They sound more work than they actually are!

The Client has requested that the section numbers in Method Statements correspond with the section numbers within Contract .......Conditions SIS-621B Attachment 10 (example) Model for health, safety and environmental method statement. As the Attachment 10 does not require sections 1.0 to 4.0 to be included in the actual method statement these sections are omitted.
EXAMPLE
5.2      SCOPE OF WORK
Cleaning of Internal and external glazing to ....................
Glazing below 45’ will be cleaned by Omnipole system utilising telescopic poles and purified water that is pumped at 60 psi through internal hose of telescopic pole to soft bristle brush the glazing will firstly be washed then gently scrubbed and finally rinsed.
High level (above 45’)  internal and external areas of canopy to station will be reached by cherry picker access equipment. Access equipment will be operated by  licensed operator. A banksman will observe and liase with access equipment operator to enable safe handling of access equipment. Communication method to be agreed between main contractor and sub contractor. once the cherry picker is in place the Omnipole method will be used to clean glazing. Omnipoles will be secured to cherry picker basket by a lanyard system complying to BS EN 362 / BS EN 354
Cleaning of Internal and external glazing to walkway.
Cherry picker access equipment will be utilised to gain access to both entrance areas the equipment will be operated by licensed operator aided by a banksman to observe and liase with the operator the glazing will be cleaned by Omnipole method. A specially designed  Omnipole will be used to clean exterior canopy to areas where it is impractical for the use of access equipment.
     .
5.2.1      Preparation The Contracts Manager will ensure all staff have read and understand the method statement and.
The station supervisor will be advised as to which areas staff will be working.
No high level ladder works are to be carried out.
The works will be carried out during Engineering hours.
Works vehicles will be safely parked.
Staff will book on and off with supervisor.
Staff will be informed of Emergency procedures and meeting point.
Staff will familiarise themselves with building layout.
Staff will inform supervisor of proposed area where work will be carried out.
Staff will carry out necessary checks on equipment and ensure certification is in order.
Hazard warning signage, cones and warning tape will be put in place prior to commencement of work to necessary areas.
Relevant hand tools, hoses and safety equipment  will be safely transported to proposed area of work.
Hoses will be enclosed in safety housing to prevent trip hazard in areas where pedestrian traffic is likely. (e.g. crossing pedestrian footway)
Staff will equip themselves with relevant PPE prior to commencement of works.
No loose objects will be carried by operatives using access equipment.
Electrical and sensitive Assets will be securely covered with plastic waterproof covers to prevent water ingress.

5.3      LABOUR FORCE
Designated site person in charge, Access equipment operator, Banksman and operators of Omnipole equipment.



5.4      TRAINING
Staff will be Physically fit and, fully trained and competent. Current station induction training will be in place for all staff and the Access equipment operator and banksman will hold industry approved certified qualifications. The ..........will hold certified training qualifications. If additional training is required it will be undertaken by ......... and or the access equipment operators.



5.5      LIFTING EQUIPMENT
See 5.7

5.6      PORTABLE TOOLS
Hand tools to be  utilised: Omnipole Telescopic window cleaning Poles

5.7      MECHANICAL PLANT ;
Powered access equipment to be hired in from a third party supplier full safety instruction will be supplied. Omnipole equipped vehicle  to be parked in safe approved area.


5.8      TRACK MOUNTED VEHICLES AND PLANT
No track mounted vehicles and plant will be used.
5.9      PROTECTION OF THE EMPLOYERS ASSETS:
Plastic sheeting will be secured over employers sensitive or electrical assets, to afford protection from water ingress.


5.10      MATERIALS
Purified De-Ionised water. This product is inert and not hazardous


5.11      STORAGE AND HANDLING OF CHEMICALS:
No chemicals will be stored on site. No Chemicals will be used to clean the glazing



5.12      SPARE

The station emergency procedures shall be observed and followed at all times by all staff whilst on site. A Fire extinguisher will be in close proximity of operatives.

No excavation work is required.
5.22      PERSONAL PROTECTIVE EQUIPMENT (PPE)
Steel toe capped boots BSN 345/6  with no exposed metal toe caps or fastenings, with good non slip soles
Goggles BS EN 166-3459B
Hi-visibility vest BS EN 471 class2
Gloves when required EN 374
Small brim safety helmet with chinstrap BS EN 397
Harness BS EN 361
Connectors BS EN 362
Lanyards BS EN 354
Waterproof clothing may be worn at discretion of operator


5.23      INCIDENTS AND REPORTING OF INJURIES, DISEASES AND DANGEROUS OCCURRENCES (RIDDOR) Incident sheets will be kept in site file. Supervisor will report to head office any incident.
Accidents and incidents including near misses will be reported to (client) and recorded in the site file. Mr ...will carry out investigation into incident. DER will be informed
     
5.24      HOUSEKEEPING
Site will be kept in orderly fashion

5.25      ACCESS AND EGRESS
By approved method and by agreed site access. Access to glazed areas will be by cherry picker (access equipment).




5.26      EMERGENCY PROCEDURES
Evacuation and Emergency procedures are to be followed as per Station Supervisors instructions.
Site supervisor will be responsible for informing all operatives on site of the emergency procedures during tool box talk and recorded in the health and safety file.

All personal will co-operate with instructions given by station supervisor.


5.27      TRANSPORTATION N/A

Warning signs – MEN WORKING OVERHEAD/ NO ENTRY, hazard tape and cones will be used to provide buffer large enough to allow for falling poles. Notices will be in place to warn other trades entering by ........

5.28      SYSTEMS/CODES OF PRACTICE
All employees to have read and understood the Method statement and Risk Assessment and COSHH assessments prior to starting task. Confirmation will be gained that all staff fully understand the task and health and safety requirements. All staff will be familiar with the site and aware of (site )procedures. A tool box talk will advise staff of the proposed work and any other issues that shift. Supervisor will use checklist to confirm all relevant matters are dealt with.


5.29      STATUTORY RECORDS
All training records are held at PH Cleaning Services offices. PH Cleaning Services Accident record book is held at Head office.


5.30      NOISE AND VIBRATION
No vibration will be caused. Care will be taken to ensure that no excessive noise is caused.


5.32      AIR QUALITY AND DUST N/A

5.33      WASTE MANAGEMENT
any waste water will be disposed of through foul water system. It is thought insignificant amount of waste water will be produced – only 60 litres per hour which will be spread over a fairly large area.

5.34      PROTECTION OF WATER QUALITY

The numbers given in the method Statement are for a particular client and should not be used, use your own numbering system. You can pick and choose the areas you need to fill in. I always give our clients the full list and put NOT APPLICABLE against the relevant areas. This method statement is for large organisations that have a Health and Safety department.

Thanks everyone for your help.

We went and gave the quote this morning with my Missis.  It took over an hour to calculate.  It was quite different from giving my normal quote; in that we were drinking coffee and troughing biscuits in a plush hotel - discussing the ins and outs with a posh guy in a suit.

We prepared.  I polished my boots and told my Missis to put some slap on!

It must have worked because he gave me a good indication that we got the job - and I wasn't the cheapest!  (They next quote was fifty quid a month more expensive - but he said he wanted quality - and didn't mind a slight difference - especially for a local firm).

He may have been palming me off though.  He said if I get a 'yes'; I'll be sent a document with a checklist of fifteen or so requirements that I have to be able to fulfill.

So it's not in the bag yet.

  Old_Master I presume the 'internal' windows just get cleaned by Omnipole with a standard squegee Not WFP or is there a method for also doing the internals with WFP.. Thanks Chris
