Office cleaning costing sheet New

Please can someone help me with a copy of cleaning costing sheet, I have been asked to quote an office for a 2hours job 5days a week .

£15 p/h = £150 p/w invoice every 4 wks

Ad extra for materials ie bin bags etc if you are to supply , or £18 p/h to include everything
