Application for payment template

I have been asked to submit an ''application for payment'' rather than an invoice. I have never submitted one of these before, can anyone point me in the right direction? or does anyone have a template i can use?

An application for payment is generally for values over 5k.

Basically you submit your application for payment - minus any VAT. This then goes to the QS in charge of works and the cost manager to sign.

You get it back and then send it in with your invoice, (plus VAT) if relevant..

I will send you a template over later because someone has sent me a wrongly filled in one. Ignore the values because they don't add up.

Hi Rob,

Thanks for that mate, really appreciate it. Yeah its for a big company and ive never even heard of it before, but i guess ya gotta learn sometime.

So regarding the two files you sent over, do i just fill out the ''application for works'' form (change all the header obviously) and send that over?

Yes

It doesn't have to be that detailed but I like clean & like to get as much info on there. If you have a purchase order number it might be worth creating a line for that as well.

You can leave my header on if you want ;D
