Do you have MS Works version 6 or higher?
If so, I can email you a template of the one I made up myself.
It's very basic.
The columns are labelled "DATE" "ITEM" "VEHICLE" "PHONE" "PRINT/POST/STAT " "TOOLS/EQUIP" "MISC" .  The next column calculates anything proportional that may be shared between business and private use (e.g. vehicle or phone) and the column after that calculates anything proportional in the "MISC" column (as things in "misc" may have various proportions).  The final column keeps a running total allowing for the apportioning.
At the top the titles are frozen so that you can see at a glance the full expendituyre of anyt column and any proportions thereof.
I made it up myself.  Like a lot of my stuff it's complex to describe but pretty easy to use once you get a bit of practice.
