anyone got/use a customer satisfaction survey

just wondering if anyone has used a customer satisfaction survey in the past that i could take a look at iv expanded quite quickly and i know that iv made mistakes etc and some customers have been left bit angry etc but it all helps to know where you went wrong so you can improve your service.iv googled customer satisfaction surveys but theyre all a bit too much!! just wondering if anyone had a generic one that i could take a few pointers from cheers

how do you rate our service.
is there any points we could make better?
how reliable do you rate us?


seems some people have to be spoon fed with everything on here

customer freedback is all you need to know and if you say some customers are not happy then you know your not providing the best possible service.

i think its overkill if your talking about domestic custys richy!

why are your custys angry? ;D ;D

is it the wfp?its frustrating in it!some come windows come up awful with it no matter how many times you rinse!

just tell them any problems and you will reclean for free as long as its in a 24 hour period.

i very rarely get custys angry though.why would they be?
price higher/work harder!

I think if a customer was not happy with the service you provided they would let you know without needing to fill out a survey.
For me I would only consider using a customer survey if I were considering expanding my services (Gutter Cleaning, Pressure Washing, Fascias etc.) and needed feedback from customers to gauge the customer demand.

i was kindof joking, but i know several of my customers are real ball-breaker types , well used to regularly giving out a tongue lashing  and most likely have a history of violence but not served long sentences
