Pre clean survey

Hi guys I am re-thinking all marketing and papper work I use to try to improve efficiencey and build a better customer database for my referals and repeat custys

My pre clean survey looks like this at the moment

Can any one recomend additional questions to make my survey more thorough

How long ago??

Was it professionaly cleaned?

What method of cleaning was used?

Hi Len I always use risk assesment forms, prety much use the same one again and again unless the premesis is radicaly different

Any chance of a copy of your risk assessment, need to fine tune mine! Wife is looking for a pay out!

Are they radically different? Think some once said surprise surprise and that’s got to be a black omen! the Met is asking some one to come home!

Cleaning Co     I might scrap all my paper work and just have a slip with that writen on, Hell in the end thats all that matters  ;D ;D ;D ;D ;D

To all those that would like my Risk assesment form I will post it as soon as ive figured out how to convert it from .wps to .pdf or .jpg (any ideas  ??? )
